RENEW Short-term and Outpatient Rehabilitation

Outpatient Therapy services including physical, occupational, nutritional, and speech therapy are available to both non-residents and residents alike. Our therapy team develops customized programs to help each patient reach their goals. Outpatient therapy requires a physician’s order.

We have designed RENEW to be the provider of choice by creating the right place to provide the right care, ensuring right outcomes at the right cost.

RENEW – Your Recovery Begins Here!

RENEW is PCSC’s innovative Short-term Rehabilitation and Post Acute Care program which focuses on restoring your independence so you can return to your address of choice. Our interdisciplinary team understands your care must be as unique as you are. PCSC professionals work with you to meet your desire for independence while providing the support you need to recover and RENEW. Areas in each community are licensed and Medicare certified. 

RENEW’s person-centered clinical, post-acute care services include:

  • Therapy coverage available seven days a week
  • Personal therapy evaluation in the first 24 hours of admission
  • Physician-driven weekly interdisciplinary rounds
  • 24/7 nursing services
  • Case Management Services: prior to, during, and post-discharge admission
  • Wellness & life enrichment programming
  • Chaplaincy services
  • Scheduled routine follow-up during first 90 days post discharge

PCSC therapy services include physical, occupational, speech, and nutritional. Additionally, aquatic therapy is available at our Foothills and The Village at Summerville communities.  

Pathways to Success

RENEW’s Pathways to Success programs are customized to your needs during short-term rehabilitation. Whether having experienced an accident, illness, injury, or surgery, these programs help restore your ability to function with greater independence.

Payment Options

PCSC communities are Medicare certified and collaborate with group payment plans, ACO’s, and hospital bundled payment programs. Most insurance providers require pre-certification and this can be coordinated between your health care provider and the PCSC community. Private pay options are also available.
